Briefing Presentation

Applications approved for Voucher Issuance must attend a briefing session describing Section 8 program policies and procedures.

 

Form must be completed by family member who is making a certification.

 

Informs participants that HUD maintains a repository of debts owed to PHAs and adverse information of former participants who have voluntarily or involuntarily terminated participation’s in one of HUD Rental Assistance Programs.

 

Form is provided at the time of a waiting list appointment.  Each family member must have one completed by the actual person unless the family member is a minor.  If the family member is a minor, then the legal guardian completes the form. Form is also provided for a participant on the program who is adding someone to their case.

Fraud is generally defined in the law as an intentional misrepresentation of material existing fact made by one person to another with knowledge of its falsity and for the purpose of inducing the other person to act, and upon which the other person relies with resulting injury or damage. Fraud may also be made by an omission or purposeful failure to state material facts, which nondisclosure makes other statements misleading.

 

Execution of the Housing Choice Voucher Program Contract (Section 8) between a property owner and a Public Housing Agency (PHA) allows the PHA to make rental assistance payments on behalf of Section 8 participating tenant.

Form is provided at the time of the Briefing Session, regarding Lead–Based Paint Poisoning.

 

Families already on the program who are interested in moving with assistance to another PHA. Participants must complete the form and provide to the PHA for review and approval.

 

Detailed list with resources to the following services: homeless services, emergency shelters, health care services, tenant protections, legal advice, fair housing and housing education, mental health, financial resources, low income housing, resources for person with disabilities, senior services, hotlines and emergency preparedness information.

 

Process to Request Reasonable Accommodation

Form provided for housing applicants and participants. Form explains the reasonable accommodation process.

Request for Reasonable Accommodation
Form provided for housing participants only.  Persons with Disabilities may request a reasonable accommodation.

 

Request to Add Live-In Aide
Form provided for housing participants only. Persons with Disabilities that require Live-in Aide assistance may submit the request by completing the Request for Reasonable Accommodation form, Verification of Disability form and Request to Add Live-In Aide form for review and approval.

 

Statement of Property Ownership
Form is completed by the property owner/authorized agent before a contract is executed.  This form is also used by property owner/agent to report address changes or management changes.

The Violence Against Women Act (“VAWA”) protects applicants, tenants, and program participants in certain HUD programs from being evicted, denied housing assistance, or terminated from housing assistance based on acts of domestic violence, dating violence, sexual assault, or stalking against them.  Despite the name of this law, VAWA protection is available to victims of domestic violence, dating violence, sexual assault, and stalking, regardless of sex, gender identity, or sexual orientation.

 

What You Should Know About EIV Form
The Enterprise Income Verification (EIV) system is a web-based computer system that contains employment and income information of individuals who participate in HUD rental assistance programs. All Public Housing Agencies (PHAs) are required to use HUD’s EIV system.

 

Zero Income Declaration
The form is usually provided to current tenants or applicants that declare to have no income.
